What they publish

electronic health records assembled for an insurer after you sign a HIPAA authorization — "Reports only exist for people who have recently applied for individual insurance coverage and signed a HIPAA authorization as part of the process"; "Milliman IntelliScript will not have a copy of your report if you have not applied for insurance in the past seven years or authorized release of information in connection with a claim." Recipient: "The insurance company you authorized to receive your information is the only recipient of your medical information."

Owner / network: Milliman, Inc. ("Copyright© 2026 Milliman, Inc."); consumer-facing brand "Milliman IntelliScript®", consumer pages live on rxhistories.com. Data gathered "from pharmacies, health insurance companies, and pharmacy benefit managers"; product lines listed on-site include Irix prescription, medical, EHR, criminal and credit data sets

Removing yourself, step by step

0 of 6 done
  1. Request your consumer report: use the "Request My Report" form at https://www.rxhistories.com/contact/, or email FCRAreport@milliman.com, or call 877-211-4816, or mail Milliman IntelliScript, P.O. Box 2223, Brookfield, WI 53008, giving name, mailing address, DOB, last 4 of SSN, the insurer that requested it, and an email if you want it emailed.
  2. Read it: "In some cases, no electronic medical data is found, so the report would be blank."
  3. To dispute, send "a written statement that identifies the disputed items from your report" by any of the same channels; Milliman "will launch an internal reinvestigation" and "may ask you to contact the corresponding provider or facility to request your own records and provide them to us".
  4. If revised, "we will send an updated copy to you and the entity that originally requested it."
  5. Trafficking victims: send documentation and proof of identity to "Attn: Trafficking Documentation, P.O. Box 2333, Brookfield, WI 53005".
  6. Going forward, read the HIPAA authorization on any insurance application — it is the only thing that lets a report be created.

How they confirm it

"Your first and last name, Your daytime contact number, Your mailing address, Your date of birth, The last four digits of your Social Security number, An email address if you want the report emailed to you, The insurance company that processed your application or claim"; "Individuals 18 years of age and older must request their own reports." Disputes: "we will require a written statement that identifies the disputed items from your report"

If they ignore you

CFPB complaint (consumerfinance.gov/complaint); state AG; FCRA §611 dispute rights; HHS OCR if HIPAA authorization scope was exceeded

Re-listing risk for this site: Low — a report exists only when you sign a new HIPAA authorization and an insurer requests it; nothing is removed so nothing re-lists
