Buyers: MIB member life/health insurers checking your application "Once you give your permission"

Removing yourself, step by step

0 of 5 done
  1. Request your MIB Consumer File, free once a year: call 1-866-692-6901 or use the online form at https://www.mib.com/request_your_record.html. If no file exists "you will receive immediate notification of NO RECORD FOUND".
  2. If you received an adverse underwriting letter citing MIB, you are entitled to "an additional copy of your MIB Consumer File at no charge".
  3. To dispute, send a signed Request for Reinvestigation Form (linked in the disclosure cover letter) or a signed letter listing each inaccurate item and why, with full name, address, DOB, place of birth and SSN, to infoline@mib.com or MIB Disclosure Office, 50 Braintree Hill Park, Suite 400, Braintree, MA 02184. "There is no charge for submitting a dispute."
  4. Include your own medical records or lab results supporting the dispute — MIB "encourage[s] you to provide us with medical records from your own healthcare providers".
  5. Expect results by mail; the process "may take up to 45 days". Removal of accurate codes is not available.

How they confirm it

for disclosure, "you will need to provide MIB some personal information so we can accurately search our files ... The identifying information you provide will be validated with other consumer reporting agencies" and by phone "you will be asked to certify under penalty of perjury that the information you provide to MIB is accurate and complete and that you are requesting a copy of your own MIB Consumer File". For a dispute: "Your full name, including middle initial ... Your address, Your date of birth, Your place of birth, Social Security Number or Social Identification Number (Canada)" plus a signed form or letter listing each disputed item

If they ignore you

CFPB complaint (consumerfinance.gov/complaint); state AG; your state insurance department; FCRA §611 reinvestigation rights; privacy questions to privacy@mib.com

Data brokers re-list. Check again in about 60 days.

Re-listing risk for this site: Low — nothing is "removed", so nothing re-lists; entries age off because reported dates "must have occurred within the past seven years", and a new individually underwritten application can create a new entry
