How to remove yourself from ExamOne

Site
examone.com
Category
risk
Difficulty
Hard — no disclosure/dispute page, no published consumer-report contact; the only documented consumer paths are lab-results access and a general contact form, and both route questions back to "your insurance company or agent"
Time to processnot published ; only figure on-site is lab results "available within 7 to 14 days subject to your insurance company's participation in this service" and "available online for one year" — that is results delivery, not an FCRA disclosure

What they publish

for the applicant's own insurer only — insurance-exam laboratory results ("As part of your insurance exam, laboratory testing will be conducted"), prescription claims ("collect the pharmacy claims regardless of where they fill their prescriptions ... mail order maintenance as well as specialty drugs, in addition to any retail prescription purchases"), "physician ordered laboratory testing results related to preventative care, disease monitoring and diagnostic purposes", and FraudCheck's "Collection accounts, the reported agency and amount due" and "Payment practices". Buyers: life insurers ("As an underwriter ...")

Owner / network: "ExamOne – A Quest Diagnostics Company" ("Copyright ExamOne, Quest Diagnostics, 2000-2026"). Products pulled from third-party data under applicant authorization: ScriptCheck® (prescription history via "some of the largest pharmacy benefits management (PBM) companies"), LabPiQture™ ("Quest Diagnostics' extensive clinical laboratory database"), FraudCheck™ Plus (identity, public records, employment and credit data "from the consumer reporting agencies on file for the applicant")

Removing yourself, step by step

0 of 4 done
  1. For your exam lab results, go to https://applicant.examone.com/, click "Register Here", select your insurer and enter the ticket number from your "Important Applicant Information" brochure; results are "available online for one year". If you lost the ticket number, use the portal's Contact Us form ("I don't know my ticket number").
  2. For a copy of any ScriptCheck, LabPiQture or FraudCheck report furnished about you, call 1.877.933.9261 (7:30 a.m.–5:30 p.m. CT) or use https://www.examone.com/about-us/contact-us/ and ask for your FCRA file disclosure; no dedicated address or form is published .
  3. Dispute inaccuracies in writing through the same channel and in parallel with the insurer that ordered the report — the site directs applicants: "If you have any questions about obtaining your results please contact your insurance company or agent."
  4. Going forward, read the HIPAA authorization in any insurance application: ExamOne states it "requires prior written consent from insurance applicants, which appears in the HIPAA compliant authorization that is part of the insurance application."

How they confirm it

portal: "Click 'Register Here' and be sure to include the ticket number on the sticker that was affixed to the front of your 'Important Information' brochure"; portal contact form asks "First Name, Last Name, Date of Birth (MM/DD/YYYY), Ticket Number, Email Address, Phone Number, Approximate Date of Exam". Requirements for an FCRA file request are not published

If they ignore you

CFPB complaint (consumerfinance.gov/complaint); state AG; FCRA §611 dispute rights; state insurance department; HHS OCR for HIPAA-scope issues

Re-listing risk for this site: Low — each report is generated per application under a signed authorization ("The authorization must be signed and dated prior to accessing the information"); nothing is removed, so nothing re-lists
