Removing yourself, step by step

1. 01

   Go to decisionlinks.com → Privacy Policy → Opt Out Form.

   If you live in CA/CO/CT/UT/VA, use the state-specific form linked there instead.

3. 03

   Fill name, street address, city, state, zip; the form also asks for SSN and DOB — leave sensitive fields blank if optional, or supply DOB only if required (do NOT supply SSN unless you accept the risk; try submitting without it first).

4. 04

   Complete the "I'm not a robot" CAPTCHA and submit; note the on-screen confirmation.
