State one qualifying condition: law-enforcement officer, identity-theft victim, risk of physical harm, or incorrect/expunged record

Include full name, DOB, aliases, current + previous addresses, phone, email

Attach redacted copy of U.S. driver's license/state ID and a printout of the listing(s) to remove; attach court orders if records were expunged/sealed

Fax to 1-888-446-1229 or mail to the Boston P.O. Box; keep copies

Follow up with support@verifyrecords.com after 30 days
Data brokers re-list. Check again in 30–60 days.
